Step 1
Understand Our Client
To obtain successful outcomes for our clients, we must first gain a deep understanding of their unique needs. We take the time to learn not only about their clinical requirements but also about their family life, personal preferences, past experiences before their injury, and their hopes for the future. This insight forms the foundation upon which we build tailored and meaningful support.
Step 2
Recruitment
We recruit specifically for each individual client based on the understanding we have gained. Each Support Worker or Health Care Assistant is interviewed against their knowledge and previous work experience of the clients condition that they have applied to support. To further personalize the selection, we create detailed profiles—often in video format—allowing clients to review and choose the team members who best align with their needs, personality and home environment prior to meeting with them face to face and making their selection. This client-centred approach fosters empowerment, leading to stronger, more successful, and long-lasting support relationships.
Step 3
The Care Plan
As recruitment progresses in the background, our focus shifts to the physical care and support we will provide. Our clinical team conducts a comprehensive assessment, reviewing the client’s injury, medical history, specific training requirements, recovery milestones, and future goals. This process includes a thorough medication management review and coordination with other healthcare professionals involved in their care. By gathering this information, we develop a care plan that not only outlines what needs to be delivered but, just as importantly, defines how it will be delivered. By working closely with the client and their Case Manager at this stage, we ensure a tailored approach that maximizes outcomes and enhances overall well-being.
Step 4
Training
Training is essential, whether we are supporting a client with a brain injury or a spinal injury. For spinal injury care, our approach is highly clinical, adhering closely to the NMC’s guidelines on delegating clinical tasks to Support Workers and Health Care Assistants. Our clinical nurses dedicate significant time working alongside the support team, providing hands-on training and continuously assessing their clinical competencies. Only when our clinicians are fully confident in the team’s skills will they transition to an overseeing role, ensuring the highest standards of care are maintained.
The training we provide to our teams supporting clients with brain injuries is highly specialized and tailored to address the unique challenges of brain injury care. See the specialist brain injury training section on our website
Step 5
Coordination & Risk Assessing
With the team recruited, trained, and the care plan in place, our care coordinators take the lead in organizing the client’s support team. They create structured rotas covering the next 12 weeks, ensuring both the client and support workers have clarity on schedules. This approach not only provides consistency for the client but also allows support workers to balance their professional and personal lives, enabling them to deliver their best care while on shift.
At this stage, comprehensive risk assessments are conducted to identify and mitigate potential hazards. These assessments ensure a safe environment for both clients and care teams, implementing appropriate safeguards to minimize risks and maintain the highest standards of care.
Step 6
Monitor & Review
With the care package now in place, our operational and clinical teams remain actively involved, working closely with both the client and their support team to ensure the highest possible outcomes. Care plans are regularly reviewed, adapted, and refined, with new goals set as previous milestones are achieved.
Clinical care packages undergo ongoing audits, with staff competencies reassessed and revalidated to maintain excellence in care delivery. Additional training is provided as needed to address emerging challenges or to support new strategies in achieving the client’s evolving goals.
